Assumption of Risks and Waiver of Rights

I understand that dancing is a sport and that there is a certain amount of risk involved. I hereby assume all of the risks, including risk of personal injury, related to my participation in the San Francisco Lindy Exchange, to be held September 13, 2012 to September 17, 2012 ("SFLX'12"). Further, I, on behalf of myself, my heirs, executors, administrators, conservators, successors and assigns, hereby waive any damage rights I may have against SFLX'12, the owners and operators of the venues, and/or any SFLX'12 organizer, staff member, volunteer or agent for any and all injury and damages I may suffer by participating in any activity which is part of SFLX'12.

Photo/Video Release

I also understand that by dancing at or otherwise attending SFLX'12, my picture may be taken or I may be filmed, and I hereby consent to the use and/or commercialization of my image (whether in print, in video, published on the Internet, or otherwise), without compensation to me, by SFLX'12, its organizers, and its or their successors and assigns, for any purpose whatsoever, including but not limited to the promotion of future dance events.
